Please complete the following form and return it with full payment for your class. Classes are limited to the first eight dogs enrolled. Your class will meet on the same day and time each week, for seven weeks, one hour per class. If multiple classes are listed, please choose one. The total cost of the class is $175.00. Since class size is limited this is only refundable if cancellation is made with John Gagnon, seven days prior to the start of the class.
